Dr. Garland will discuss the outcomes and biobehavioral mechanisms of Mindfulness-Oriented Recovery Enhancement (MORE), an evidence-based therapy for addiction, emotion dysregulation, and chronic pain. The current opioid crisis has been driven in part by an epidemic of physical and emotional pain. Unfortunately, there are few empirically supported interventions that can simultaneously treat this complex comorbidity. To meet this need, Mindfulness-Oriented Recovery Enhancement (MORE) was generated through a decades-long treatment development process. Rooted in affective neuroscience, MORE unites mindfulness training, cognitive-behavioral therapy, and principles from positive psychology into an integrative therapy designed to simultaneously target addictive behavior, emotional distress, and chronic pain. MORE’s efficacy has been demonstrated in 16 clinical trials to date. In the largest NIH-funded trial of MORE, MORE decreased opioid misuse by 45% by 9-month follow-up, nearly tripling the effect of standard group therapy. MORE has also been shown to prevent drug relapse while significantly decreasing craving, pain intensity, depression, and PTSD symptoms. Neuroscientific investigations indicate that MORE’s treatment effects are mediated by restructuring reward responses, strengthening emotion regulation, and modulating self-referential processing in the brain. These mechanisms represent novel treatment targets for optimization trials combining MORE with neurofeedback, neuromodulation, and psychedelics.
